As far as the problem with your plant I've been studying Mr. Cannucks he says your water Ph is off when this happens either too high or too low you wanna keep it between (6.2% – 6.8%) 6.5 is perfect.Buy a ph level tester if possible

Try germinating directly in the soil with a heat mat and a dome to retain the moisture. I have had 100% germination on my last 10 cycles.
Do u apply foliarly or you add the fertilizers to the soil
